Are you running out of Eggs?(Dimished Ovarian Reserve)

In spite of you having normal cycles, the ovaries could be running out of the eggs
that may be available to you to conceive. We need to suspect this condition if you
are unable to conceive in a reasonable frame of time (Reduced Fecundity),
Inadequate response to the hormones given to you by your doctor or unexplained
repeated pregnancy losses, wherein urgent attention is required and aggressive
management to prop up the reduced ovarian reserve and also to bail you out of
opting for donor eggs.

Causes can be Idiopathic (where no cause is identified as yet), Chemotherapy,
Radiotherapy, Genetic mutations, Ovarian (Endometriosis)and tubal surgeries, etc.
Smoking, which researchers say tends to propone menopause by a couple of years
and hence one may be running out of eggs earlier than same age woman.
Since nearly 10% of a woman undergo accelerated oocyte (egg) pool depletion,
which leads to infertility early on, and may affect many women delaying
childbearing, this issue may be of concern since it means many more women will
end up in a Diminished ovarian reserve. So the knowledge of ovarian reserve would
lead women to make childbearing decisions accordingly.
There are many ways you can be helped out provided we catch the situation in
time.

3As (Age, AMH, AFC) is the way to go ahead to check your ovarian reserves.
AGE---- Normally, the number of eggs at birth is around 1-2 million which stands
at around 4, 00,000 at puberty and slips down to 25,000 to 35,000 at 35 years of
age. A gradual decline in number corresponds with the reduction in fertility
potential according to age. But this process can be accelerated in a few cases
needing attention.
AMH (Anti Mullerian Hormone)– This is a blood test to assess your ovarian reserve.
Your doctor will Interpret the values according to your age and plan the further
fertility protocol tailor-made for you.
AFC(Antral follicle count)-This is the ultrasound assessment of ovary. Usually the
transvaginal ultrasound is done on the 3rd day of menses to know the number of
follicles in your ovary which are capable of growing further to yield the eggs. The
lower number points towards the Diminished reserves.
If you are at all concerned about your fertility, it is best to speak to a doctor, who
will be able to help you to determine your chances based on the ovarian reserve
tests, age and how you have done in your previous attempts to conceive and give
your individualized treatment protocol.
